Abstract

Objective: Compare bone marrow transplantation versus umbilical cord blood transplantation in terms of HLA compatibility and graft-versus-host disease. Method: Systematic literature review in PubMed, Scopus, and Google Scholar during October-November 2024. The following terms were used: bone marrow transplantation, umbilical cord blood, HLA compatibility, and graft-versus-host disease. Fifteen articles published between 2019 and 2025 were included. Results: Bone marrow requires 8/8 HLA compatibility, while umbilical cord blood tolerates 4/6 compatibility without a significant increase in mortality. The incidence of acute GVHD grades III-IV was 15-20% in cord blood versus 30-40% in bone marrow. Chronic GVHD reached 25-35% in cord versus 40-60% in bone marrow. Acute kidney injury affected 45-60% of cord recipients. Conclusions: Umbilical cord blood offers lower HLA compatibility requirements and reduced incidence of graft-versus-host disease, expanding access to transplantation.
