Objective: Presentation of a case of polyarticular juvenile idiopathic arthritis with systemic manifestations and resistance to conventional treatment. Method: Clinical case report of a 14-year-old adolescent diagnosed with JIA at the age of 10. Results: The patient developed systemic manifestations (fever, rash), symmetrical arthritis in the wrists, knees and hands, prolonged morning stiffness, mild joint deformities and elevated inflammatory markers. She had a suboptimal response to conventional treatment with methotrexate and corticosteroids, highlighting the need to escalate therapy to biological agents. Conclusions: The case illustrates the clinical heterogeneity of polyarticular JIA, highlighting the importance of differential diagnosis with other autoimmune pathologies and the relevance of multidisciplinary management to optimise functional prognosis.
