Objective: to analyse the surgical and non-surgical management of secondary and primary hyperparathyroidism. Method: Systematic review. Results and conclusion: The management of both primary and secondary hyperparathyroidism should be based on a comprehensive and personalised evaluation that considers the severity of the disease, the patient's comorbidities and the available therapeutic options. Parathyroidectomy is consolidated as an effective intervention in serious cases, offering significant benefits in metabolic control and the prevention of complications, although non-surgical strategies, such as the use of cinacalcet, represent viable alternatives in patients not suitable for surgery, technological advances, such as microwave ablation and improved surgical techniques, promise to optimise clinical results.
