Objective: to analyse access to neonatal metabolic screening in rural areas of Santo Domingo de los Tsáchilas, Ecuador. Method: descriptive observational study of 51 newborns who underwent neonatal metabolic screening in 2023. Results: Economic limitations, reported by 21.7% of families, highlight the need to implement specific measures to guarantee equitable access to neonatal metabolic screening. Similarly, restricted access to medical services in rural areas (52.2%) and geographical and transport barriers (56.5%) indicate the importance of strengthening healthcare infrastructure and improving accessibility. Conclusion: Access to neonatal metabolic screening in rural areas such as Alluriquín, Santo Domingo de los Tsáchilas, is limited by economic, geographical, cultural and communication barriers, compromising the coverage and effectiveness of this essential service.
